These care models are offered separately, allowing patients to choose the option that best fits their needs and preferences.

Dr. Catherine Chokuba-Azum is a Family Medicine physician, healthcare leader, and founder of Azum Medical, a Houston-based practice created with a simple vision: to make exceptional healthcare feel personal, compassionate, and accessible.
With experience caring for patients across diverse clinical settings, Dr. Azum believes that excellent primary care begins with listening. Her approach goes beyond treating a diagnosis— she takes the time to understand the whole person, identify individual health goals, and partner with patients in making informed decisions about their care.
